Document requirements vary per program. However, the below listed documents are typically required for all children, unless extenuating circumstances exist.

  • Child's Birth Certificate
    • To replace a lost or stolen Birth Certificate, parents/guardians may use this resource.
  • Social Security Card
    • To replace a lost or stolen Social Security Card, parents/guardians may use this resource.
  • Up-to-Date Shot Records (on the Universal State of Louisiana Record with the seal present)
    • To view a sample of this document and more information on how to obtain this information, parents/guardians may use this resource.
  • Proof of Residence
    • Typically, a current gas, water, or electric bill is required to prove residence in Lafayette Parish.
    • Some programs do not require this or allow other documents to be submitted for residence verification.
  • Proof of Income for all adults living in the household
    • Each program will specify which types of income documentation are allowed based on the household income scenario.

The Online Application Process

Q. Will I be able to apply for more than one site?

Parents and/or guardians may submit one application indicating their first and second choice, depending on the grade level and the program.

Q. How long does it take to complete the online application process?

Typically, the process should take from 10-20 minutes, depending on the program you are applying for.

Q. What if I do not have access to a computer?

Any electronic device with internet access will be applicable. Each site will also have computers available for parents. For Lafayette Parish School System, the Vermilion Conference Center will be the only location that will have computers available.

Q. Is there another way of submitting an application other than online?

No, the application process is strictly electronic.

Q. Once I've submitted the application electronically, how will I know that the submission is successful?

Once the application is completed online, you will receive a generated message based on the email address you provided when you created an account.

Q. Should I allow anyone else to use my account?

It is the user's responsibility to protect the integrity of any information entered in the system. This includes but is not limited to passwords or personal information about you or your child. You must also agree not to share your login information or password with anyone except a parent or legal guardian.

Q. My account has been disabled because I've attempted to login too many times. Should I set up another account?

No. You may use the link to reset your password through email.

Q. After I have completed the online application process application, is there another step to complete the process?

Yes. You will receive an email and/or letter with further instructions.

Q. Once I've applied and received the email, what types of proof or documents will I need to submit?

Q. Can I change my first-choice selection even though my application has already been submitted to a school?

Changes to an application can be made by logging into the website.

Q. I missed this year's application deadline. Will I have a chance to apply at another time?

Applications are on-going throughout the year but deadlines are determined by sites.
